A Race Against Time: Saving a Critically Injured Patient in District Orakzai

25-year-old Salahuddin from UC Sheikhan in Village Zukhani, District Orakzai, was brought to the Emergency Department at DHQ Hospital Mishti Mela in critical condition following a severe firearm injury. He arrived in shock, fighting for his life, with multiple serious internal injuries.

The emergency team acted swiftly, stabilizing the patient and preparing him for emergency Laparotomy. Despite the challenges of managing such a critically injured patient in a remote area with limited resources, the surgical, anesthesia, and nursing teams worked together to provide life-saving care without delay.

During the surgery, the situation became even more critical when the patient’s heart suddenly stopped. The surgical and anesthesia teams responded without delay, performing life-saving resuscitation and successfully restoring his heartbeat. Doctors discovered extensive injuries to several vital organs and severe internal bleeding. The team carried out emergency procedures to control the bleeding, repair life-threatening damage, and prevent further complications. Multiple blood transfusions were provided throughout the surgery to support the patient’s survival.

Recognizing that the patient would require highly specialized intensive care and advanced diagnostic facilities unavailable locally, the team performed essential life-saving measures to stabilize him and safely prepare him for transfer. After nearly three hours of continuous emergency care and surgery, the patient was successfully referred to a Tertiary Care Hospital in Peshawar for further treatment.

This case highlights the dedication, skill, and resilience of healthcare professionals working in remote and underserved areas. Managing such a complex and life-threatening emergency is a significant achievement and demonstrates the growing capacity of local health services to provide timely, high-quality care when every minute matters.
